Senior LIFE Uniontown Expanding To Offer More Care For Fayette County Seniors

Senior LIFE is a Program of All-Inclusive Care for the Elderly (PACE). The program provides medical, healthcare and personal support services for seniors who want to remain living in their home, but need support services to do so. All services are provided at no cost to those who qualify. In Pennsylvania, the program is called LIFE.

Senior LIFE Uniontown opened December 2009 and provides Medicare and Medicaid-eligible seniors a comprehensive healthcare program so that they can remain living in their homes and communities. Participants in the program pay no costs; have no deductibles, copays, and no cost for prescription drugs.

Senior LIFE has experienced tremendous growth in the last six years with over 242 members. The program, which is funded by Medicare and Medicaid, brings together a coordinated team of doctors, nurses, social workers, dieticians, therapists and home health aides to provide seniors with an individualized care plan and coordinated health and personal care services that address their unique medical care and social needs.

The Senior LIFE Center is expanding an additional 3,600 square feet to allow more services for the members who visit Monday through Friday. This additional space will accommodate 76 more members and features extended day room area for memory-impaired members, 6 additional bathrooms, an expanded gym and therapy area, extension to conference room space, an additional exam room, and additional room for the clinic.

Natalie Martin, Senior LIFE Uniontown Executive Director, oversees a growing member base as well as a growing staff. She attributes the success of the Uniontown program to the staff: “We have a great staff. I’m surrounded by people who really care about the members. With this addition in space, we will be able to take care of more members in need and space for our staff to utilize their care.”

The services are provided both in the Senior LIFE Center located at 89 W. Fayette Street, and in the participant’s home. Previous to Senior LIFE Uniontown, the building was originally a Midtown Foodland. This expansion will continue to allow the center to continue to grow to the expanding need of services in Fayette County. Designed to allow medically eligible seniors to remain in their homes independently, Senior LIFE provides meals, comprehensive healthcare and therapy services, social and recreational activities, full clinical care, social services and personal care.

Joe Angell, a current member of Senior LIFE, comes to Senior LIFE three days a week, and receives physical therapy on those days for pain management. He has been very pleased with the care he receives for his pain management, medication, and the attentiveness of Dr. Stumpf and the medical team; he would recommend the program to anyone: “There are no out-of-pocket costs for me, and to receive care like this without worrying about money has made all the difference.”

The program also offers in-home healthcare care services, including personal care, home delivery of prescription drugs and light housekeeping. The LIFE Program provides comprehensive support services that enable seniors to remain independent in their homes and live in the community rather than in nursing homes. Senior LIFE also provides transportation to and from the LIFE Center and all medical appointments as well as an escort to the medical appointments. Transportation is provided in comfortable, climate-controlled vehicles specifically designed to safely accommodate wheelchairs, walkers and oxygen equipment. The transportation is provided at no cost to the member, and to the great relief for family caregivers.

To be eligible for Senior LIFE services, seniors must be 55 years of age or older, reside in Fayette County, be determined medically eligible and able to be safely served in the home and community. Senior LIFE’s support services allow family caregivers to maintain their personal and professional commitments while securing safe, friendly care for loved ones. Senior LIFE Uniontown currently serves the entire Fayette County. There is no cost for the services for those who qualify.
